It usually is. Not many are competent enough to pierce or tattoo themselves with confidence.

Yes. It's a part of fashion.

Very common. Some piercings more than others.

Do they hold different meanings too?

In some cultures. From what I've read it could indicate cultural or social status, coming of age, independence, belonging, etc. Some religions forbid it for some stupid as hell reason like it's "desecrating God's creation" (a person's body).

That last reason seems very... narrow minded.

It is. If someone wants to believe that about themselves that's fine. People should be allowed to do what they want to their own bodies. It's just annoying to force others into that.
